Long-Term Care
One Vaccination Program, Two Distinct Populations
Three long-term care organizations used ThereCare to manage flu, pneumococcal, and broader vaccination programs across multiple facilities, applying different strategies to residents and employees.

Population-specific vaccination management across multiple facilities
The Challenge
Long-term care organizations are responsible for populations with very different vaccination needs. Residents may be older, medically vulnerable, and receiving care within the facility, while employees are a mobile workforce who may receive healthcare elsewhere and require a different approach to engagement and vaccination. Managing both populations across multiple locations adds further complexity: establishing who belongs to each population, determining vaccination history and needs, coordinating services, documenting vaccinations, and maintaining visibility across the organization.
How ThereCare Was Used
ThereCare enabled the organizations to manage residents and employees as distinct populations within a single vaccination program.
- Uploaded resident and employee rosters in bulk instead of requiring individual self-registration
- Stratified individuals by organization, facility, population type, and other program-defined criteria
- Queried IIS records for residents and staff to understand vaccination history and identify additional needs
- Organized facility-based vaccination workflows around residents' clinical operations
- Managed employee vaccination through a separate, workforce-oriented approach
- Deployed on-site vaccination teams once six or more employees needed vaccination
- Documented vaccination encounters at the point of service
- Tracked vaccination information by individual, population, facility, and organization
The Outcome
The organizations moved beyond treating vaccination as a series of disconnected clinics. ThereCare let them establish each population, understand vaccination history, identify needs, select an appropriate intervention, deliver services, document encounters, and maintain visibility across multiple facilities — turning "who wants a flu shot?" into "what does this population need, and how should we deliver it?"

What This Demonstrates
ThereCare can stratify a single vaccination program by population — applying facility-centered care for residents and workforce-oriented, demand-based clinics for employees — while still measuring coverage and ROI across the whole organization.
